DESIGN AND IMPLEMENTATION OF CHILD IMMUNIZATION MANAGEMENT SYSTEM (A CASE STUDY OF OLOMOWEWE CLINIC OFFA, KWARA STATE) – Complete Project Material

Child immunization is a critical public health initiative aimed at protecting children from vaccine-preventable diseases. Despite its importance, the immunization process at OLOMOWEWE Clinic in Offa, Kwara State, relies on manual record-keeping, which is prone to inefficiencies, errors, and data loss. This project focuses on the design and implementation of a child immunization management system to address these challenges by automating immunization records, scheduling, and reporting.

The study employed a systematic methodology, including requirements gathering through interviews and observations, system design using Unified Modeling Language (UML) tools, and development using PHP, MySQL, HTML, and JavaScript. The resulting system features a centralized database for secure record storage, automated notifications to remind parents of upcoming vaccinations, and reporting tools to monitor immunization trends and coverage.
